A bit about this role: This position is an amazing opportunity for an ambitious early-mid careerpharmacist to join a high performing pharmacy team. You'llwork closely with Devoted's clinical leadership team in Waltham, MA to provide pharmacysupport to relevant aspects of our Part D and care management programs. At the same time, you'llwork with Devoted Medical Group (DMG) to help deliver home-based primary care to the frailest, sickest, and most complex members of our health plan. You'llsupport Devoted's clinical team and clinical guides - a cadre of expert social workers andregistered nurses - to help with our medication reconciliation program for members transitioning to home from the hospital. You'll also play a central role in ensuring our members take the right medication at the right time, driving better clinical outcomes through better medication adherence and educating them about opioid medications. What we need: A pharmacist with 3 or more years of clinical experience (ideally supporting efforts to care for medically and socially complex populations) An appreciation of Medicare regulations and policies and how Medicare pays for drugs A keen understanding of how the interactions between payers, providers, and drug manufacturers ultimately affect the patient Experience with pharmacy-related quality assurance (QA) work is strongly preferred but not required (examples include experience performing medication reconciliation during transitions of care or helping to design and/or implement other pharmacy-related quality improvement programs). Experience working as part of a multidisciplinary care team, especially during transitions of care.
